Homebound instruction is provided for students who are attending a Milan Special School District. It is intended for students who will be unable to attend school for 2 consecutive weeks or longer due to medical conditions. There must be strong medical justification for this restrictive placement.
Homebound services are not a disciplinary placement or a place for students to make up missed assignments. It is not a setting to help students regain missing credits. It is important to remember that students may in some cases, return to school still behind in their class-work since often students are seriously ill and cannot maintain the pace of the classroom. It is a last resort for students who are unable to attend school due to medical or emotional needs. In order to determine if a student is eligible for homebound services, the parent needs to complete an application process. The Application Process
Parents must ensure that both the completed application form for homebound signed by a physician and a release of information form signed by the parent are submitted to the Cathy Moore at Central Office. Contact information for faxing or mailing these documents is provided on the forms. The application forms include the current medical diagnosis and treatment plan, supported by documentation from the office visits and/or hospitalization admission and discharge summary. Once we receive both the completed application and release of information documents, the district’s Homebound Service Supervisor will review it.
If a determination of eligibility is made, parents will receive a letter stating that the student is eligible, the date the services begin and end, and the homebound teacher will be identified. You would then receive a phone call from the homebound instructor to set up their first visit. On approval of homebound services, please be aware that students will be prohibited from participating in after school activities or working an after school job unless an exception form is completed and approved by the District.
Once homebound services are begun, the homebound teacher is responsible for collecting assignments, providing them to the student, reviewing questions and lessons the student has, and then returning the completed work to the school teacher. Any outstanding or unfinished work remaining after homebound services are terminated becomes the student’s responsibility. Homebound services are provided in the home with a responsible adult present at all times. Students are expected to adhere to the same School Board Policies as if they were attending school. Services for most students are typically 3 hours per week which is completed in two visits during each week.
